Medication Therapy Management (MTM)

Medication Therapy Management (MTM) is not a benefit but a free service that is being offered by Retiree RxCare. The MTM program helps to make sure you are using medications that work best to treat your medical conditions. It can also help us identify possible medication errors.  

If you meet the criteria, we will automatically enroll you in the program and mail you information. Participation is voluntary. You can opt out at any time, and we will remove you from the program.  

When you receive MTM services, the offered 1- on- 1 review of your medications with a pharmacist or other trained healthcare provider at no cost is called a Comprehensive Medication Review (CMR).  Additionally, if an issue is identified with your medications that requires attention, you or your prescribing provider may be contacted about a Targeted Medication Review (TMR).  Other outreach attempts may be required if an issue during a CMR or TMR is identified that requires your prescribing provider’s attention. The prescriber may be contacted by telephone or fax from our MTM provider.

In order to qualify for these services, you must meet all the following targeting criteria:
 

The 2025 criteria

1. Be an At-Risk Beneficiary or
2. Meet ALL of the following criteria:
   1. Have at least 3 of the following conditions or diseases: Alzheimer’s Disease, Bone disease-arthritis (including osteoporosis, osteoarthritis, and rheumatoid arthritis), Chronic Heart Failure (CHF), Diabetes, Dyslipidemia, End-Stage Renal Disease (ESRD), Human Immunodeficiency Virus/Acquired Immunodeficiency Syndrome (HIV/AIDS), Hypertension, Mental Health (including depression, schizophrenia, bipolar disorder, and other chronic/disabling mental health conditions), or Respiratory Disease (including asthma, chronic obstructive pulmonary disease (COPD), and other chronic lung disorders), AND
   2. Take at least 8 covered Part D medications, AND
   3. Are likely to have medication costs of covered Part D medications greater than $1,623 per year.

If you meet all the above criteria, Retiree RxCare will invite you to enroll in our MTM program. You will receive a comprehensive enrollment packet in the mail.
